Drug-induced liver injury (DILI) is one of the most challenging adverse drug reactions in clinical practice and new drug development; it is the leading cause of acute liver failure in Europe and the United States, and its incidence has been on the rise in Asia in recent years. With the widespread adoption of multidisciplinary treatment approaches and the advent of an ageing society, the complexity of patients’ drug exposure has increased, leading to continuous evolution in the aetiology, clinical phenotypes and diagnostic and treatment pathways of DILI. This study systematically synthesises recent research advances in DILI, providing an in-depth discussion on epidemiology, risk factors and pathogenesis, diagnosis, treatment and prognosis, as well as the specific characteristics of the elderly population and precision prevention and control. It aims to review the latest developments in drug-induced liver injury research, explore its mechanisms, diagnostic and therapeutic strategies in depth, and provide a reference for future academic research.
